refactor: discover-agent 改为按需加载（disable-model-invocation: true）

Browse Source 
与 Claude Code 的 skills 加载机制对齐：默认不将完整内容注入 system prompt，
改为通过 Skill 工具按需加载，节省 token 消耗。
